Is the procedure painful, and what kind of anesthesia is used?

At 6venus, your comfort is our priority. TESA is usually performed under local anesthesia or mild sedation, and most men describe it as a minor “pinch” or pressure. Micro-TESE is a more detailed microsurgery, so it is performed under general anesthesia or deep sedation. This ensures you are completely asleep and feel no pain during the 1–2 hour procedure. Post-surgery, you may feel some dull aching or soreness, which is easily managed with the personalized pain-relief plan provided by Dr. Pavan Bendele.

This is a common question. Unfortunately, surgically retrieved sperm are usually fewer in number and do not have the same “swimming ability” (motility) as naturally ejaculated sperm. Because of this, they cannot travel through the cervix and uterus effectively on their own. Therefore, these sperm must be used with ICSI (Intracytoplasmic Sperm Injection), where a single healthy sperm is injected directly into the egg in our lab to ensure the highest chance of fertilization.

We believe in complete transparency. While Micro-TESE has the highest success rate for finding sperm (up to 60% in many cases), there is a chance that no viable sperm are found. Before the surgery, Dr. Pavan Bendele will discuss “Plan B” with you. This might include using donor sperm (if previously agreed upon) or discussing further medical therapies to improve production before a future attempt. Having an on-site lab allows us to give you a definitive answer while you are still in the facility.

This is a major concern for many men. Micro-TESE is specifically designed to be “tissue-sparing.” Because Dr. Bendele uses a high-powered microscope, he only removes the tiny tubules that appear healthy, rather than taking large chunks of tissue. This precision protects the Leydig cells (which produce testosterone). Most studies show that testosterone levels remain stable or return to normal shortly after the procedure, and there is no long-term impact on sexual function or libido.

Success starts long before the surgery. Dr. Bendele often recommends a 3-month “1% Natural” preparation phase. Since it takes about 72–90 days for new sperm to mature, we suggest:

  • Stopping smoking and limiting alcohol.
